TOURNAMENT REGISTRATION:

Time: 6:30 – 9:30 pm


Location: Marriott North (West Chester), 6189 Mulhauser Road, West Chester, OH 45069
Directions: Take I-75 to exit 19 and go west on Union Center Blvd. The hotel will be on the right just past
the exit.

It is mandatory for all teams to register on Friday evening. One of your parents or another coach from your club may register your team.

What to bring to Team Check-In:

  1. A copy of your official stamped and validated team roster to leave with the tournament. Rosters are frozen as of registration on Friday evening.
  2. Validated Player Pass Cards for both rostered players and guest players.
  3. Validated Coaches Cards, one for each coach. Maximum of 3 coaches.
  4. Travel Permit: Teams from outside Ohio South are required to provide an approved travel permit from their respective state association.
  • This includes both Kentucky and Indiana teams participating in Cincinnati area league play.
  • For Ohio South teams, go to the OSYSA website and click on Forms and Resources, then the E-Travel
  • For Kentucky teams, go to the KYSA website click on Administrative and E-Travel
  • For Indiana teams, got to the Indiana Soccer website and click on Services-Forms
  • For all other teams, please check with you respective state association.
  1. Guest Player Roster approved and validated by your respective state association.
  • The maximum number of guest players for U8-U9 teams is (2), U11-U12 teams is (3) and U13-U19 teams is (4).
  • A registrar will be available on site to process guest player passes for Ohio South teams only. These players will receive a player pass that is good for the duration of the tournament. The following are required at team check-in to obtain a pass:
    • ​A signed guest player registration form
    • A copy of the player's birth certificate
    • A 1.25"x1.25" photo of the player
    • $7 for OSYSA processing fee
  1. Medical Release – We will not check medical release forms; however, we highly recommend that you have them throughout the tournament.
  2. There is no liability form to complete for the tournament.
